CarePlan Membership Terms

Effective October 16, 2026

These terms apply to every CarePlan membership and service bought on getcareplan.com from CarePlanUSA LLC. The plan you choose, its price, its term and its cancellation rules are shown in full in the Membership Agreement you read and sign before you pay. CarePlan is a healthcare membership, not health insurance, and does not cover emergency care. If you are in danger, call 911.

1. What a membership includes
Each plan covers a set number of doctor visits, video visits, lab tests and specialist visits for each 6-month period, as listed on the plan and in your agreement. Visits are scheduled with CarePlan's network of providers in New York.
2. Payments
Memberships are billed to the card you provide: at sign-up, then every month, or in advance for the 12-month prepaid option. A platform service fee of 5.5% of the plan price is added to each charge. Some plans include a one-time First Visit Fee, shown before you pay.
The immigration evaluation is paid in two payments: a deposit at sign-up and a final payment before the report is released.
Payments are processed by Stripe. CarePlan never sees or stores your full card number.
3. Cancellation and refunds
You may cancel at any time with written notice to info@getcareplan.com. Cancellation takes effect at the end of your current billing period, and nothing is charged when you cancel.
- Month to month: no long-term commitment. Refund requests are reviewed by our staff.
- 6-month term: if you cancel before the term ends and later come back, your first payment back includes the difference between the month-to-month rate and your plan price for each month you paid, plus the 5.5% platform service fee on it.
- 12 months prepaid: If you leave before the 12 months end, we refund the months that have not started; the First Visit Fee and the free month are not refunded.
The platform service fee is not refundable.
4. Rules and conduct
Members are expected to treat all staff and healthcare providers with respect. Abusive behavior, missed appointments without notice, or fraudulent use of membership benefits may result in termination of membership. Members must provide accurate health information and follow recommended treatment plans.
5. Liability waiver
CarePlan provides healthcare services to the best of its ability. By enrolling, you acknowledge that medical outcomes cannot be guaranteed and that you release CarePlan, its employees, and affiliates from liability for any adverse outcomes arising from treatment, except in cases of gross negligence or willful misconduct.
6. Privacy and data
Your personal and health information will be handled in accordance with HIPAA regulations and our Privacy Policy. We may use your contact information to send appointment reminders, health tips, and important membership updates. You may opt out of non-essential communications at any time.

7. Governing law
This agreement shall be governed by and construed in accordance with the laws of the State of New York. Any disputes arising from this membership shall be resolved through binding arbitration in New York County, New York.
